Delegate Scholarship Program: District 29C

The questionnaire/application provides the scholarship selection committee for Delegate Gerald “Jerry” Clark with the necessary information to justify awards of scholarships.  Failure to complete this questionnaire will result in disqualification of  consideration.  Answer all questions completely.  For questions that do not apply, please mark “N/A” or “not applicable”.  All information disclosed in the questionnaire  remains confidential.

Requirements:

• must plan to attend 2-4 year undergrad institute or trade/career school
• must be enrolled n a maximum of 6 credit hours
• must reside in Delegate Clark’s District of 29C (to verify visit Maryland General Assembly Website )  Select the icon labeled “Who Represents Me” located at the right of the screen, and enter your address. If one is not a resident of District 29C, the application will not be processed
• completed FASFA must be attached to application
• most recent transcripts must be attached to application
• a cover letter must be attached to application
